a. information about the applicant, including full name, gender, and date of birth.<\/p>\n

b. information about the planned visit, including the date of arrival and departure, and the place of visit.<\/p>\n

c. information about the invitee, including name, contact number, address, official seal, and signature of the legal representative of the entity or the invitee.<\/p>\n

5. Other supporting documents<\/p>\n

For those who do not apply in their country of nationality, proof of legal residence or residence status is required. In some cases, applicants may be required to submit a letter of proof from their employer with the following details: the name and contact details of the employer, the applicant’s income statement or the original copy of the applicant’s bank statement for the last six months.<\/p>\n
